Travel Release Form Information

In order for a parent to transport an athlete to / from a meet you must complete the following form and turn the waiver into the Head Coach one day prior to the event. North Park Runners can submit this form to Coach Neff and Practice Squad runners can turn this form into their specific coach.

Athletes are not permitted to travel to and from meets on their own.
